ADOPTION PROCESS
Please complete this form, as well as our adoption application, and submit it to IMPACT for Animals.
The adoption fee for a purebred dog begins at $200.00 and a mixed breed dog is $150.00. The adoption fee for cats is $100.00 if they are purebred and / or declawed the fee is $150.00. Adoption fees do vary based on the costs of our medical fees. The adoption fees help cover the costs of spays, neuters, vaccinations, other vet services, food and foster supplies.
All dogs are current on shots including, rabies, DHLPP and bordetella. Each has been tested for heartworms and currently on heartworm preventative. Puppies receive DHLPP’s and wormings as appropriate for age. Adult dogs have been spayed or neutered. Puppies will need to be spayed or neutered between the ages of 4 and 5 months at one of our affiliated veterinarians.
Cats have tested negative for FeLuk and FIV. Adult cats have rabies and FVCRP vaccinations, have been spayed or neutered. Kittens receive FVCRP vaccinations and wormings as age appropriate. Kittens will need to be spayed or neutered between the ages of 4 and 6 months.
Additional kitten and puppy shots including rabies are the responsibility of the new owner upon adoption.
IMPACT for Animals reserves the right to refuse adoption of any animal to anyone.
ADOPTION AGREEMENT
By adopting a pet you understand and commit to:
All pets must be spayed or neutered prior to adoption or if a puppy or kitten within the fourth to sixth month of their life. Part of your adoption fee has paid for this service through one of our affiliated vets. We reserve the right to reposes your puppy or kitten if you do not comply with our spay/neuter policy.
If, for any reason or at any time, I am unable to care for my adopted pet, I agree to contact Impact for Animals to return it to them or offer them the first right for placement.
I further agree never to abandon my adopted pet or deposit my adopted pet at an animal shelter or research facility; however, before returning my adopted pet to IMPACT for Animals, I agree to exhaust all options available to me including, but not limited to, obedience training, a behaviorist, crate training or veterinary advice to solve a physical or behavioral problem that may occur with my adopted pet or between me and my adopted pet.
I agree that by accepting my pet for adoption, I am accepting ownership of the animal. I understand this means that IMPACT for Animals is not responsible for any damage to person or property that may be caused by my adopted pet after the date of the adoption and I accept full responsibility for my adopted pet from the date of the adoption forward.
